What the research says

A
Strong human evidence
Why: Approved osteoporosis drug: large RCTs (1,637 and 1,360 women) and meta-analyses of dozens of RCTs consistently show fewer vertebral fractures.

Teriparatide (PTH 1-34) is an approved osteoporosis drug backed by large human randomized trials and multiple meta-analyses showing it reduces vertebral fractures and raises bone density, including head-to-head superiority over risedronate in severe osteoporosis. The rat osteosarcoma signal has not been borne out in large human registry studies, though more people stop treatment because of side effects than on placebo.

    Authors' conclusion
Bisphosphonates, denosumab, abaloparatide, teriparatide and romosozumab followed by alendronate reduce clinical fractures in postmenopausal women with osteoporosis; abaloparatide and teriparatide increased withdrawals due to adverse events.

Meta-analysis of 23 head-to-head RCTs (6,680 patients): teriparatide lowered fracture risk vs bisphosphonates (RR 0.61) and raised spine and hip bone density, with no overall increase in adverse events.

    Key resultTeriparatide reduced fracture risk vs bisphosphonates (RR = 0.61, 95% CI 0.51-0.74, P < 0.001); denosumab showed no fracture benefit (RR 0.99, 95% CI 0.62-1.57, P = 0.96)

    Authors' conclusion
Teriparatide was superior to bisphosphonates in decreasing fracture risk, and teriparatide and denosumab both increased BMD more than bisphosphonates without more adverse events.
    Limitations

Teriparatide: questions people ask

What is Teriparatide?

Parathyroid hormone analog. The active first 34 amino acids of parathyroid hormone; daily injections stimulate new bone formation.

How much does Teriparatide cost?

The cheapest Teriparatide from an independently tested vendor is $1.50 per mg (Andy Peps Studio, TER10 Teriparatide 10mg * 10 vials, $150.00). Across 9 listings from 7 vendors the median is $3.05 per mg.

Where can I buy Teriparatide?

7 vendors tested by Finnrick sell Teriparatide. The cheapest per mg right now: Andy Peps Studio ($1.50), Baohua Dongnuo Biotechnology BHD ($1.50), LN Peptides ($1.50). 0 of them publish a lab report (COA) for Teriparatide; compare lab scores and reports in the table above before buying.

How much bacteriostatic water for 10 mg of Teriparatide?

Adding 2 mL of bacteriostatic water to a 10 mg vial of Teriparatide gives 5 mg/mL: each 10 units on a U-100 insulin syringe holds 500 mcg. With 1 mL it's 10 mg/mL (1,000 mcg per 10 units); with 3 mL, 3.333 mg/mL (333.3 mcg per 10 units). More water makes small amounts easier to measure. Mixing charts: 10 mg.

Is Teriparatide FDA approved?

FDA-approved as Forteo for osteoporosis.

Can Teriparatide be compounded in the US?

FDA-approved (Forteo and others) for osteoporosis. Compounding status for every peptide.

Does Teriparatide work? What does the research say?

Evidence grade A (strong human evidence), from 8 papers: Approved osteoporosis drug: large RCTs (1,637 and 1,360 women) and meta-analyses of dozens of RCTs consistently show fewer vertebral fractures. See the papers.

Are there clinical trials of Teriparatide?

ClinicalTrials.gov lists 152 registered trials of Teriparatide: 15 active, 106 completed, 33 in phase 3, 56 with posted results. Examples: Study of the Key Techniques of Prevention and Treatment of Osteoporotic Refracture (Recruiting, 2,478 people); To Evaluate the Efficacy and Safety of Abaloparatide Injection (QLG2128) in the Treatment of Postmenopausal Women With Osteoporosis and at High Risk of Fracture (Not yet recruiting, Phase 3, 282 people); Preoperative Teriparatide Use on Insertional Torque of Pedicle Screws for Lumbar Fusion Surgery (Active, not recruiting, 192 people).

What dose of Teriparatide was used in studies?

Teriparatide is an approved drug. Forteo: 20 mcg subcutaneously once a day into the thigh or abdomen; use for more than 2 years in a patient's lifetime only considered if fracture risk remains or returns to high; postmenopausal women with prior vertebral fractures (Neer et al.): 20 or 40 microg parathyroid hormone (1-34) vs placebo once daily; postmenopausal women with severe osteoporosis and prior vertebral fractures (VERO, double-dummy vs risedronate): 20 μg teriparatide (vs risedronate 35 mg weekly) once daily. These are amounts used in published studies or official labels, not a recommendation.

What are the side effects of Teriparatide?

Reported in the research: Nausea; Headache; Dizziness; Stopping treatment because of adverse events (more than placebo); Calciphylaxis (painful skin/vessel calcification). Approved prescription drug; rat studies showed bone cancer at high lifetime doses, but large human registry studies have not found an increased osteosarcoma rate.
