Animal studies: Mouse endurance studies (e.g. PMID 18674809) used daily injections over about 4 weeks; that 'exercise mimetic' use has not been tested in people.
All human studies gave AICAR (acadesine) intravenously in clinical or laboratory settings; none tested it for fitness, fat loss or by subcutaneous injection. Not an approved drug.
What the research says
AICAR (acadesine) has been given to humans mainly as an intravenous drug during heart surgery and in small metabolic and cancer studies, with mixed results: an early pooled analysis suggested fewer heart attacks, but a large later trial found no benefit. Its popular use as an 'exercise mimetic' rests on mouse data; there are no human trials of it for fitness or fat loss.

Effect of acadesine, a new metabolic agent, on exercise-induced myocardial ischemia in chronic stable angina.Humans12 peopleRandomizedPlacebo-controlledDouble-blindFive-way randomized double-blind crossover-type comparison of four acadesine doses with placebo
Randomized crossover study in 12 people with stable angina: IV acadesine at four doses did not improve time to ischemia, angina or exercise time versus placebo.
Key resultPlacebo-adjusted change in time to 1-mm ST depression was -0.1 ± 6.2%, 11.1 ± 13.8%, 12.9 ± 8.6%, -3.2 ± 6.8% at 6, 12, 24, 48 mg/kg (p = NS vs. baseline)

- Time to 1 mm ST-segment depression and exercise tolerance
- Sample

Intravenous AICAR administration reduces hepatic glucose output and inhibits whole body lipolysis in type 2 diabetic patients.HumansTen male type 2 diabetic patientsIntravenous AICAR vs saline infusion with stable isotope methodology and muscle biopsies
In 10 men with type 2 diabetes, IV AICAR reduced liver glucose output and lowered blood glucose and free fatty acids compared with saline.
Key resultGlucose disposal as percent of appearance increased and plasma glucose fell more with AICAR (both p < 0.001); plasma NEFA appearance and disappearance were reduced

- Compared with
- NaCl 0.9% control test
- Main outcome
- Glucose and fatty acid kinetics
- Authors' conclusion
- IV AICAR reduced hepatic glucose output and lowered blood glucose in type 2 diabetes, and may stimulate hepatic fatty acid oxidation and/or inhibit lipolysis.

Intravenous AICAR during hyperinsulinemia induces systemic hemodynamic changes but has no local metabolic effect.HumansTen participants (paired data n = 8)Placebo-controlled120-minute clamp, AICAR over last 60 minutesTwo euglycemic hyperinsulinemic clamps with AICAR or placebo infusion
In 10 healthy volunteers, IV AICAR during an insulin clamp did not change whole-body or forearm glucose uptake but raised heart rate and lowered blood pressure.
Key resultNo difference in glucose infusion rate or forearm glucose uptake; heart rate rose from 58 ± 3 to 70 ± 3 vs 60 ± 4 to 63 ± 4 bpm with placebo (P < .05)

- Compared with
- Saline placebo
- Main outcome
- Whole-body glucose infusion rate and forearm glucose uptake
- Authors' conclusion
- AICAR does not seem to have a direct effect on systemic or local glucose uptake in humans; it increases heart rate and decreases blood pressure, probably by systemic vasodilation.
- Limitations
- Small sample.

AICAR: questions people ask
What is AICAR?
AMPK activator (nucleotide analog). Switches on AMPK, the cell's energy sensor, mimicking some effects of exercise on muscle metabolism in animal studies.
How much does AICAR cost?
The cheapest AICAR from an independently tested vendor is $0.118 per mg (Andy Peps Studio, AR50 AICAR 50mg * 10 vials, $59.00). Across 23 listings from 20 vendors the median is $0.480 per mg.
Where can I buy AICAR?
20 vendors tested by Finnrick sell AICAR. The cheapest per mg right now: Andy Peps Studio ($0.118), Baohua Dongnuo Biotechnology BHD ($0.118), LN Peptides ($0.118). 0 of them publish a lab report (COA) for AICAR; compare lab scores and reports in the table above before buying.
How much bacteriostatic water for 50 mg of AICAR?
Adding 2 mL of bacteriostatic water to a 50 mg vial of AICAR gives 25 mg/mL: each 10 units on a U-100 insulin syringe holds 2,500 mcg. With 1 mL it's 50 mg/mL (5,000 mcg per 10 units); with 3 mL, 16.667 mg/mL (1,666.7 mcg per 10 units). More water makes small amounts easier to measure. Mixing charts: 50 mg.
Is AICAR FDA approved?
Not approved. Prohibited in sport (WADA).
Does AICAR work? What does the research say?
Evidence grade B (some human evidence), from 8 papers: Large RCTs exist only for heart-surgery protection and conflict (3,080-patient trial negative); 'exercise mimetic' use rests on mouse data and tiny human studies. See the papers.
Are there clinical trials of AICAR?
ClinicalTrials.gov lists 1 registered trials of AICAR: 0 active, 1 completed. Examples: Contraction (Exercise) Mediated Glucose Uptake as a Therapeutic Target in Type 2 Diabetes (Completed, 40 people).
What dose of AICAR was used in studies?
AICAR is studied in people. intermediate- to high-risk adults undergoing on-pump coronary bypass surgery (RED-CABG): 0.1 mg/kg per minute for 7 hours, plus acadesine added to cardioplegic solution single perioperative infusion; adults undergoing coronary bypass surgery (Multinational Acadesine Study): 0.1 mg/kg per minute over 7 hours, plus 5 micrograms/ml in cardioplegic solution single perioperative infusion; patients with relapsed/refractory chronic lymphocytic leukemia (phase I/II): single doses of 50-315 mg/kg; then 210 mg/kg given 2 or 5 times single or repeated 4-hour infusions. These are amounts used in published studies or official labels, not a recommendation.
What are the side effects of AICAR?
Reported in the research: Raised blood uric acid; Transient low blood pressure during infusion; Increased heart rate; Transient anemia or low platelets; Kidney impairment. Human safety data come from IV hospital use (heart surgery, leukemia, metabolic studies); there are no safety data for self-administered use.
