Doses studied in research Studied in people
What was given to people (or animals) in published studies and official labels, each linked to its source. This is not a recommendation: study products, purity and supervision differ from research-grade vials.
| Who was studied | Dose given | How often | For how long | What happened | |
|---|---|---|---|---|---|

0.5 or 1.6 mg/mL topical (applied to the ulcer) | not stated in abstract | not stated in abstract | no significant improvement in healing vs placebo in the full population; post hoc benefit suggested only in ulcers of at least 10 cm2; well tolerated | PubMed |
| adults with hard-to-heal venous leg ulcers (first-in-human, randomized, double-blind, placebo-controlled) n = 34 |
0.5, 1.6 or 3.2 mg/mL topical (applied to the ulcer) | twice weekly | 4 weeks plus 4-week follow-up | faster healing at 0.5 and 1.6 mg/mL vs placebo; no difference at 3.2 mg/mL; no local or systemic safety concerns | PubMed |
Human trials used LL-37 applied directly to chronic wounds in research formulations; no injected-dose trials for other uses were found. A 2023 diabetic foot ulcer trial (cream twice weekly for 4 weeks) did not state the concentration in its abstract. Not approved by FDA or EMA.
What the research says
LL-37 is the human cathelicidin antimicrobial peptide; most literature is lab and animal work on its antimicrobial and immune roles. A few small human trials of topical LL-37 for chronic leg and foot ulcers exist: an early trial and a diabetic-foot trial looked positive, but the largest (148-patient phase IIb) trial found no overall healing benefit over placebo.

Evaluation of LL-37 in healing of hard-to-heal venous leg ulcers: A multicentric prospective randomized placebo-controlled clinical trial.Humans148 peopleRandomizedPlacebo-controlledDouble-blindPhase IIb double-blind, randomized, placebo-controlled multicentre trial (HEAL LL-37) of topical LL-37 (0.5 or 1.6 mg/mL) plus compression
Phase IIb trial in 148 patients with hard-to-heal venous leg ulcers: topical LL-37 plus compression did not improve healing vs placebo overall; a post hoc subgroup with large wounds did better. Well tolerated.
Key resultNo significant improvement in healing with LL-37 vs placebo in the full population; a post hoc analysis showed significant improvement in several healing parameters in patients with wounds of at least 10 cm2.
Study details
- Studied
- Patients with hard-to-heal venous leg ulcers (mean age 67.6 years, median ulcer duration 20.3 months)
- Compared with
- Placebo
- Main outcome
- Ulcer healing
- Sample
- 148 patients in three arms
- Authors' conclusion

Treatment with LL-37 is safe and effective in enhancing healing of hard-to-heal venous leg ulcers: a randomized, placebo-controlled clinical trial.Humans34 peopleRandomizedPlacebo-controlledDouble-blind3-week run-in, 4-week treatment, 4-week follow-upFirst-in-man randomized, double-blind, placebo-controlled dose-response trial after an open-label placebo run-in
First-in-human trial in 34 venous leg ulcer patients: the two lower LL-37 doses healed faster than placebo (significant for the lowest dose), but the highest dose showed no difference; no safety concerns.
Key resultHealing rate constants for 0.5 and 1.6 mg/mL LL-37 were about six- and threefold higher than placebo (p = 0.003 and p = 0.088); mean ulcer area fell 68% (0.5 mg/mL) and 50% (1.6 mg/mL).

LL-37 Might Promote Local Invasion of Melanoma by Activating Melanoma Cells and Tumor-Associated Macrophages.Humans + animalsImmunohistochemistry plus in vivo and in vitro experiments
LL-37 levels in human melanoma rose with tumor stage, and LL-37 stimulated melanoma cells and macrophages to make pro-invasion, pro-angiogenic factors in cell and mouse experiments, a possible safety concern.
Study details
- Studied
- Melanoma tissue from patients (immunohistochemistry), B16F10 mouse melanoma, A375 human melanoma cells and M2 macrophages
- Main outcome
- LL-37 expression vs T stage; pro-angiogenic and invasion factor expression
- Authors' conclusion
- LL-37 expression correlated positively with melanoma T stage and stimulated tumor cells and macrophages to induce pro-angiogenic factors, suggesting LL-37 might promote melanoma invasion. A safety signal for LL-37, not a treatment study.
- Limitations
- Mostly preclinical; correlation in tissue.

LL-37: questions people ask
What is LL-37?
Human antimicrobial peptide. The only human cathelicidin: kills bacteria, fungi and some viruses and helps wound healing, but can also drive inflammation at high levels.
How much does LL-37 cost?
The cheapest LL-37 from an independently tested vendor is $1.50 per mg (Andy Peps Studio, LL375 LL37 5mg * 10 vials, $75.00). Across 75 listings from 56 vendors the median is $7.20 per mg.
Where can I buy LL-37?
56 vendors tested by Finnrick sell LL-37. The cheapest per mg right now: Andy Peps Studio ($1.50), Baohua Dongnuo Biotechnology BHD ($1.50), LN Peptides ($1.50). 6 of them publish a lab report (COA) for LL-37; compare lab scores and reports in the table above before buying.
How much bacteriostatic water for 5 mg of LL-37?
Adding 2 mL of bacteriostatic water to a 5 mg vial of LL-37 gives 2.5 mg/mL: each 10 units on a U-100 insulin syringe holds 250 mcg. With 1 mL it's 5 mg/mL (500 mcg per 10 units); with 3 mL, 1.667 mg/mL (166.7 mcg per 10 units). More water makes small amounts easier to measure. Mixing charts: 5 mg, 10 mg.
Is LL-37 FDA approved?
Not approved. Research use only.
Can LL-37 be compounded in the US?
Removed from Category 2 in April 2026; FDA plans to put it to its compounding advisory committee before the end of February 2027. Until FDA adds it to the 503A bulks list, US pharmacies can't compound it from bulk. Compounding status for every peptide.
Does LL-37 work? What does the research say?
Evidence grade B (some human evidence), from 8 papers: Three small topical RCTs in leg and foot ulcers conflict: early and diabetic-foot trials looked positive, but the largest (148 patients) found no healing benefit. See the papers.
Are there clinical trials of LL-37?
ClinicalTrials.gov lists 5 registered trials of LL-37: 0 active, 4 completed, 1 with posted results. Examples: Intratumoral Injections of LL37 for Melanoma (Completed, Phase 1, Phase 2, 4 people); Salivary TAS, TOS, LL-37 and Dental Status in Passive Smoking Children (Completed, 180 people); Effects of Smoking and Vitamin D3 on the Levels of Human Cathelicidin Peptide LL-37 (Completed, 60 people).
