Animal studies: Rat studies used single intranasal doses of 50-250 microg/kg body weight, or 0.15 mg/kg intraperitoneally.

Semax is registered as a medicine in Russia but is not FDA- or EMA-approved. Human studies are small, mostly non-randomized Russian trials; other studies (optic nerve disease, healthy-volunteer fMRI) do not state the dose in the abstract.

What the research says

Mostly animal studies

Why: Human data are small non-randomized, unblinded Russian studies; the controlled evidence comes from rat and mouse experiments.

Semax (an ACTH(4-10) analogue) has been tested in small Russian clinical studies (open-label or without described randomization) in stroke, optic nerve disease, motor neuron disease, psoriasis and atopic dermatitis; no large placebo-controlled RCTs are in the candidate literature. Most other evidence is rat gene-expression and behaviour work, so benefit in people is plausible but not firmly established.

Evidence grade C (mostly animal studies), from 11 papers: Human data are small non-randomized, unblinded Russian studies; the controlled evidence comes from rat and mouse experiments. Semax (an ACTH(4-10) analogue) has been tested in small Russian clinical studies (open-label or without described randomization) in stroke, optic nerve disease, motor neuron disease, psoriasis and atopic dermatitis; no large placebo-controlled RCTs are in the candidate literature. Most other evidence is rat gene-expression and behaviour work, so benefit in people is plausible but not firmly established. See the papers.

What dose of Semax was used in studies?

Semax is studied in people. adults in the acute period of hemispheric ischemic stroke (vs 80 conventionally treated controls): 12 mg/day (moderate stroke) or 18 mg/day (severe stroke) daily; adults after ischemic stroke in early or late rehabilitation, semax vs no-semax subgroups: 6000 mcg/day daily, in 2 courses of 10 days with a 20-day interval; patients with motor neuron disease (open-label): 12 mg daily (1% solution) daily. These are amounts used in published studies or official labels, not a recommendation.
