Q: How much bacteriostatic water for 10 mg of SS-31?

Adding 2 mL of bacteriostatic water to a 10 mg vial of SS-31 gives 5 mg/mL: each 10 units on a U-100 insulin syringe holds 500 mcg. With 1 mL it's 10 mg/mL (1,000 mcg per 10 units); with 3 mL, 3.333 mg/mL (333.3 mcg per 10 units). More water makes small amounts easier to measure. Evidence grade B (some human evidence), from 8 papers: Several RCTs with conflicting results: the 218-person phase 3 myopathy trial and a heart-failure trial failed; benefit in Barth syndrome is mainly open-label. SS-31 (elamipretide) has been tested in several human RCTs. Results are mixed: the phase 3 trial in mitochondrial myopathy and a heart-failure trial missed their primary endpoints, while Barth syndrome patients improved mainly in open-label extensions, supporting a 2025 US accelerated approval for that rare disease. Evidence is for specific diseases, not general anti-aging use. See the papers.

Elamipretide (SS-31) is approved only for Barth syndrome. Trials in mitochondrial myopathy and heart failure did not meet their primary endpoints at the doses studied.

Some human evidence

Why: Several RCTs with conflicting results: the 218-person phase 3 myopathy trial and a heart-failure trial failed; benefit in Barth syndrome is mainly open-label.

SS-31 (elamipretide) has been tested in several human RCTs. Results are mixed: the phase 3 trial in mitochondrial myopathy and a heart-failure trial missed their primary endpoints, while Barth syndrome patients improved mainly in open-label extensions, supporting a 2025 US accelerated approval for that rare disease. Evidence is for specific diseases, not general anti-aging use.

Adding 2 mL of bacteriostatic water to a 10 mg vial of SS-31 gives 5 mg/mL: each 10 units on a U-100 insulin syringe holds 500 mcg. With 1 mL it's 10 mg/mL (1,000 mcg per 10 units); with 3 mL, 3.333 mg/mL (333.3 mcg per 10 units). More water makes small amounts easier to measure. SS-31 (elamipretide) has been tested in several human RCTs. Results are mixed: the phase 3 trial in mitochondrial myopathy and a heart-failure trial missed their primary endpoints, while Barth syndrome patients improved mainly in open-label extensions, supporting a 2025 US accelerated approval for that rare disease.
