Q: How much bacteriostatic water for 10 mg of TB-500?

Adding 2 mL of bacteriostatic water to a 10 mg vial of TB-500 gives 5 mg/mL: each 10 units on a U-100 insulin syringe holds 500 mcg. With 1 mL it's 10 mg/mL (1,000 mcg per 10 units); with 3 mL, 3.333 mg/mL (333.3 mcg per 10 units). More water makes small amounts easier to measure. Mixing charts: 2 mg, 5 mg, 10 mg, 20 mg, 25 mg.

Q: Does TB-500 work? What does the research say?

Evidence grade D (little or no evidence), from 11 papers: Only one rat study tests TB-500 itself; the human trials and most animal work used full-length thymosin beta-4, a different molecule. TB-500 is a synthetic version of a fragment of thymosin beta-4; essentially all published human data are small trials of full-length thymosin beta-4 applied topically (eye drops for dry eye and neurotrophic keratopathy, a gel for venous ulcers), with modest or borderline results. There are no published human trials of injected TB-500 itself; the rest is animal and cell work, mostly on full-length thymosin beta-4, with only one rat study of TB-500 itself. See the papers.

TB-500 is studied in people. patients with stage 2-3 neurotrophic keratopathy (phase III, placebo-controlled); full-length thymosin beta-4 (RGN-259), not TB-500: 0.1% thymosin beta-4 ophthalmic solution eye drops (frequency not stated in abstract); healthy volunteers (randomized, placebo-controlled phase I); synthetic full-length thymosin beta-4: 42, 140, 420 or 1260 mg single dose, then the same dose daily for 14 days; healthy Chinese volunteers (randomized phase I); recombinant human thymosin beta-4 (NL005): single doses of 0.05-25.0 micrograms/kg; multiple-dose cohorts 0.5, 2.0 or 5.0 micrograms/kg single dose, or once daily for 10 days. These are amounts used in published studies or official labels, not a recommendation.

Animal studies: In rats with repaired Achilles tendons, TB-500 was given at 60 micrograms/kg/day intraperitoneally for 4 weeks (PMID 42542926).

All human studies used full-length thymosin beta-4, not the TB-500 fragment sold online; no human trial of injected TB-500 itself was found. A venous-ulcer gel trial (PMID 20536470) tested topical concentrations including 0.03%. Not an approved drug.

What the research says

Little or no evidence

Why: Only one rat study tests TB-500 itself; the human trials and most animal work used full-length thymosin beta-4, a different molecule.

TB-500 is a synthetic version of a fragment of thymosin beta-4; essentially all published human data are small trials of full-length thymosin beta-4 applied topically (eye drops for dry eye and neurotrophic keratopathy, a gel for venous ulcers), with modest or borderline results. There are no published human trials of injected TB-500 itself; the rest is animal and cell work, mostly on full-length thymosin beta-4, with only one rat study of TB-500 itself.
